Volunteer Application Volunteer Release Authorization Form Volunteer Disclosure Form Volunteer Discloure Form (Copy) Student Leadership Team Application To volunteer, complete all of the following forms and turn into the Central Kids offices at your earliest convenience. The disclosure copy is for your records unless requesting a copy of your background check. The Student Leadership Team Application is for anyone under the age of 18 who is interested in serving in our ministry. A background check is not required for the Student Leadership Team application.
